Extra-renal production of 1α,25(OH)2D3 has been reported in granulomatous states, but in critical illness no clear data exist. We have previously reported in patients undergoing cardiopulmonary bypass (CPB) that 1α,25(OH)2D3 overshot significantly above baseline, a few days later. The mechanism of this rise and whether it is sustained were unclear.
