Abstract Timely identification of patients who will die in the foreseeable future is essential to providing appropriate care, but such identification is difficult. This study aims to identify evidence-based early identification tools for life limiting illness in the primary care setting, and to describe their core elements. Four diagnostic tools were identified. A narrative process described and compared the identified tools with a focus on practicality in clinical care. The tools identified pave the way for the development of an international, standardized, evidence-based approach to the early identification of palliative patients.
