We read with interest the case report by Brod C and co-workers published on April 24, 2009 in the Journal der Deutschen Dermatologischen Gesellschaft.

In this paper the authors report on their management approach to a patient with “dysplastic” nevus syndrome and multiple melanomas. In an attempt to reduce the patient’s melanocytic burden, which theoretically may remove any unrecognized melanomas and facilitate follow-up examinations, the authors embarked of surgically excising large segments of the patient’s nevus involved skin and grafting the defects with normal skin devoid of nevi.

This management approach is quite unusual and warrants further discussion.

In conclusion, we are of the opinion that “prophylactic” surgical excision of benign nevi offers little to no benefit to patients at risk for developing melanoma since it does not prevent future melanomas from developing nor does it eliminate the need for periodic surveillance examinations. The utilization of total body photography and/or digital dermoscopic follow up offers patients a practical and reasonable alternative to “prophylactic” surgery.
