Formatted abstract Acute respiratory tract illness (ARTI) is a leading cause of human morbidity worldwide and is most 
often viral in origin. Human rhinoviruses (HRVs) are the most frequent pathogens in ARTI and have 
been associated with lower respiratory tract illness (LRTI), especially in the young, those with 
asthma and other vulnerable populations. Classified within the family Picornaviridae, the genus 
Rhinovirus consists of 100 serotyped‐strains divided into two species, Human rhinovirus A (HRV A; 
n=75) and Human rhinovirus B (HRV B; n=25). Unlike other respiratory viruses however, HRVs are 
generally not considered as individual entities with distinct circulation patterns so the presence 
and role of individual HRV strains in respiratory illness remains largely unexplored.  
 
ARTI studies involving polymerase chain reaction (PCR)‐based detection methods have, since 1994, 
produced unusual HRV‐like sequences or made cursory reports of untypeable or variant HRV 
strains. More recently, an investigation of picornavirus PCR products obtained from ARTI cases 
during 2003 identified a novel clade of the genus Rhinovirus, designated HRV A2. The HRV A2 clade 
was populated by more than 30 nucleotide sequences demonstrating limited sequence identity 
with any picornavirus sequence on GenBank. The complete polyprotein coding sequence of one of 
these putative viruses was deduced and phylogenetic analysis identified this sequence as a novel 
HRV strain, entitled HRV‐QPM. This virus was the first new HRV strain to be described in two 
decades and serves as the prototype of the HRV A2 viruses.  
 
To determine whether HRV‐QPM was a distinct respiratory pathogen endemically circulating in the 
Brisbane population, a comprehensive retrospective molecular epidemiology study was 
undertaken on a well‐defined specimen population obtained predominately from paediatric 
patients with symptoms of ARTI. HRV‐QPM was detected at a prevalence of 1.4% by two novel 
real‐time reverse‐transcriptase PCR assays. Prevalence peaked in winter but was also high in 
spring and summer. HRV‐QPM was most frequently detected in infants with expiratory wheezing 
or persistent cough, usually associated with admission to hospital which was often accompanied 
by a requirement for supplemental oxygen and it was the only virus detected in 65% of HRV‐QPM 
positive individuals. These observations resulted in an objective clinical impact ranging from mild 
to severe. This was the first molecular epidemiology study to address the annual prevalence and 
clinical impact of an individual HRV strain.Despite repeated attempts in typically HRV‐susceptible cell lines, isolation of HRV‐QPM in culture 
was unsuccessful. The age and amount of viable virus contained within the inoculum was 
proposed as a major reason behind the culture failings. However, HRV A2 strains may be 
particularly fastidious and employ receptors that are not present on cell‐lines currently classified 
as HRV‐susceptible. The inability to isolate members of the HRV A2 clade has important 
implications for current HRV culture techniques, antiviral development and classification schemes.  
 
During 2007, the complete coding sequences of six previously unknown and highly divergent HRV 
strains were reported. HRV‐QPM was the initial strain described. ‘HRV‐Xs’ were subsequently 
found in the United States and ‘HRV‐Cs’ were described in Hong Kong. Collectively these putative 
viruses occupied a distinct position within the genus Rhinovirus, previously identified as HRV A2. 
Because it appeared that the members of this clade were frequently associated with LRTI, this 
study sought to better characterize them in the hope of finding features that might prove useful in 
predicting the outcome of HRV A2 infection.  
 
To catalogue the molecular features distinguishing the divergent HRV strains, this study undertook 
computer‐based in silico analyses of all available HRV and representative picornavirus polyprotein 
sequences including intensive genomic analysis and protein homology modelling of the HRV A2 
prototype strain, HRV‐QPM, which resulted in comprehensive characterization of all HRV A2 
polyprotein sequences. By superimposing the sites of known receptor contacts onto predicted 
HRV‐QPM proteins and predicting HRV A2 strain drug susceptibility, this study was able to propose 
a more robust taxonomic placement for these newly identified viruses and suggest some novel 
features that may be useful in future taxonomic developments. The divergent HRV strains did not 
meet the classification requirements for any existing species of the genus Rhinovirus or the closely 
related genus Enterovirus. HRV A2 strains should be partitioned into at least one new species, 
putatively called Human rhinovirus C.
